How many times as this happenedReally I don't even know how to deal with something like this, but often there's not a lot you can do in a situation like this.  Maybe a different approach next time, but interesting nonetheless.

How do you get it right in the end though? Is it enough to just blindly follow the steps involved without thinking about what you are trying to achieve? My observations show that this just doesn't always work. There's another element involved that is often needed to make it work don't you think?
